LPN - Licensed Practical Nurse - MCH/FASD - Community Health
Description

The LPN will work in the Maternal Child Health-FASD Department. This position includesassessments, mentoring, health teaching and linking with community resources. With an emphasis on in-home support to families in their community. This positing is an integral part of MCH, FASD and Community Health programs. The LPN will also be the case coordinator for the FASD Diagnostic Team.

Reports to: MCH Supervisor

Responsibilities
  • Provide holistic assessment and counseling to all self-identified pregnant women.
  • Be a strong advocate for the prevention of FASD.
  • Initial assessments of the newborn and the post-partum women will be done in collaboration with community health nurse and/or supervisor.
  • Administer appropriate screening tools and make necessary referrals.
  • Follow-up with infant and family as required.
  • Prepare and present classes specific to the needs of the family, working with other team members when needed.
  • Encourage healthy family lifestyle practices at every opportunity.
  • Use approved developmental assessment tools for screening and determining referrals.
  • Link parents with appropriate education and supports.
  • Act as a source of information for breastfeeding and nutrition throughout the years.
  • Provide Health promotion activities in the schools, working with community health as needed.
  • Complete diagnostic reports. Follow up with recommendations made by team.
  • Connecting with and referring to agencies/health professionals on and off reserve
  • Document all encounters in the appropriate health record.
  • Complete a monthly planning calendar and report to the MCH Supervisor at the end of each month.
Qualifications
  • Licensed practical Nurse- SK registration and eligible for AB registration.
  • Current CPR; Strong computer skills.
  • Effective communication skills: Ability to work independently and as a member of a team.
