What is Home Care?
- Helping people help themselves
- Services for people of any age; mainly in their home
- Help people to be able to stay in their own home and be with their loved ones as long as possible
- Work with other service providers to get the needed care

What Home Care Is Not:
- Replacing care provided by family
- Maid service
- Taxi/Transportation service
- 24 hour care
- Medication delivery service
- Do not administer oral medication without Doctors orders
- Do not make appointments for your regular doctor (family responsible)
Who Can Get Home Care?
Persons of any age who have an assessed need who:
- Have just come home from hospital
- Waiting for care in a long term care home
- Have an illness or disease that need follow-up
- Need nursing care in the house
- Choose to live at home instead of a long term care home as long as its safe
- Family and friends who need help to continue to care for person at home

- Registered Nurses
- Licensed Practical Nurses
- Certified Home Health Aides
- Personal Care Aides
How Can I Get This Service?
- You may refer yourself
- Referred by others (family, friends, your doctor)
- Referred by hospital
Home Care Schedules:
Home Care Nursing is provided daily
8:00am to 4:30pm – Monday to Friday
Weekends are covered, as per schedule as needed
Onion Lake Home Care Services Includes:
Client Assessment
Assessment by the home care registered nurse assessor within 72 hours after receiving a referral
- Talking with you and your family
- Talking with your doctor and other people who have cared for you
- Reviewing your health history
- A physical assessment
- Assessment of clients’ needs
- Appropriate referrals, communication with other providers
- Care planning to meet goals
- Skilled intervention
- Teaching
- Coordination of services
Case Management
- After your assessment, case manager plans for your care that is right and is provided by the right care giver at the right time
- Review of care
- Evaluation and documentation of clients response to care
- Efficient use of resources
- Admission and discharge planning
- Care plans are continuously updated based on reassessments, every six (6) months to once per year
Home Care Nursing
- Nursing care that is provided in your home or community setting
- Teaching you self care
- Teaching your family to help you care for yourself
- Supervising the workers who provide you with the personal care
Wound Care Monitoring
- To tend to dressing changes as per doctors’ order
Medication Management
- Monitoring for therapeutic intervention
- To administer medications as per doctors’ orders
- Client advocacy
Foot Care
- Individuals unable to do own foot care due to decrease vision or impaired mobility will receive foot care
- Monitors early signs of problems to prevent further problems
Mental Health
- To monitor medications and mental status
- To administer medications as per doctors’ orders
- Client advocacy
Palliative Care
- To maintain clients’ optimum comfort and care
- To provide nursing care and support to client and family in the home
- Administer medications as per doctors’ orders
- Client advocacy
Home Support
- Help with light housekeeping
- Laundry
- Meal preparation
- Help with activities like bathing
- Foot care
- Helping get dressed
In-Home Respite Care
- Caring for you when it is not safe for you to be left alone while your family, who usually care for you, has a rest
